Registration will be on a first-come, first-served basis,
since sessions fill up quickly (many by the end of February). Phone

On-Line
registration is handled for Technique Swim Camp by thriva.com.
Costs
for on-line registration are $425 for a 5 day camp and $340 for a 4 day camp.
You
will need to provide emergency contact information, the name of
your health care provider and your insurance carrier during the
registration process.
While
on-line registration is fast and easy, your child’s space is NOT guaranteed until we receive a copy of his/ her physical performed
within the past two years. The copy of the physical should arrive
at our office within one week from the day registered. Since space
is limited, if the copy of the physical does not arrive within
one week, the application will be considered "incomplete" and
there is a chance that your child may lose his/her space.
The
copy of the physical must include the following: proof of the
child’s physical within the past two years that is clearly dated
and signed by the physician and specifically grants approval for
the child to participate in sports camps; a legible copy of immunization
history including a recent TB test and Hepatitis B shot. You may
use the linked Medical

Registration:
If you do not want to register on-line, you can register by mail. However, please be advised that we cannot guarantee that a week will be open by the time we get your registration form. It is highly possible that an on-line registration will close a week between the time you mail your registration form and the time we receive it.Please make sure you have checked our session availability table

by mail and the session you request is full by the time your
form is received, you will be called to decide on a second
choice, or placed on a wait list.

Credit
Cards are not accepted for mail registration. Upon receipt of
the registration form, medical forms (Part A and Part B) and payment,
an email confirmation will be sent.
Both
parts of the Medical Form must be completed. Part A is
to be completed by the parent/guardian. Part B requires proof
that the child has had a physical after August 13, 2008.
The

Incomplete
medical forms will be returned and campers may forfeit their space
in camp.
Note:
Checks that are returned for insufficient funds will be subject
to a $25 fee. If you are registering more than one child or are
registering for more than one session, special payment arrangements

Cancellation
Policy
If
you cancel a session before May 1 and the session still has
availability, then you will receive a refund minus a $25 administrative
fee.
If
you cancel a session before May 1 and the session is closed
to further registrations, then you will receive a refund minus
a $25 administrative fee if the space can be filled. If the
space cannot be filled, then you will receive a refund minus
$100 for holding a space in a full week as we will have turned
business away.
If
you cancel a session after May 1 and the session still has availability,
then you will receive a refund minus a $100 administrative fee.
If
you cancel a session after May 1 and the session is closed to
further registrations, then you will receive a refund minus
a $100 administrative fee if the space can be filled and
the session is cancelled before 5:00 PM on the Wednesday prior
to the enrolled session. If the space cannot be filled,
then the full amount is forfeited.
Prescription
Medications (12
kb PDF file): Authorization to administer prescription medication
to the camper while at camp.
Transportation (13 kb
PDF file): Designates specific persons allowed to transport camper
from camp. This is required for car-pools or any drivers other
than legal guardians.
Adobe
Acrobat Reader is required to read .pdf files. If you don't have
this software, you can download it for free from Adobe's
web site.
